You may opt in to receive messages in the following ways:

1. Online Form (below): Enter your information and check the consent box to agree to receive SMS notifications.
2. In person / by phone: When scheduling an appointment, you may request SMS reminders. Our staff will explain that messages may be stopped at any time by replying STOP, and HELP will provide support details.
3. Text-in keyword: You may text the word SUBSCRIBE to our phone number. You will receive an automatic confirmation reply:

“JAC Computer Services: You are now opted in to receive reminder and notification messages. Reply HELP for help, STOP to opt out. Msg & data rates may apply.”

Your phone number will only be used for sending reminder and notification messages from JAC Computer Services. We will not share or sell your information.
